Sunday Health and Five Star Home Health Care Partner to Improve Cognitive Care for Virginians Living With Mild Cognitive Impairment and Dementia

ALEXANDRIA, Va. - March 31, 2025 - PRLog -- Many Virginians living with subjective cognitive decline, mild cognitive impairment (MCI), and Alzheimer's disease and related dementias (ADRD) struggle to access timely diagnoses and specialized care, particularly in rural areas of the Commonwealth. Today, nearly 164,000 Virginians aged 65 and older live with ADRD. According to the Virginia Dementia State Plan, about 300,000 Virginians over 45 have reported worsening changes in memory or thinking—often an early indicator of cognitive decline.

To address these challenges, Sunday Health and Five Star Home Health Care have partnered to expand access to expert cognitive evaluation, early intervention, and long-term support.

Sunday Health is a specialty clinic focused on proactive cognitive care, offering preventive programs, timely clinical evaluations, and a longitudinal care model via telehealth or in-person in Vienna, VA. Its team of cognitive neurologists, geriatric nurse practitioners, and care navigators provides personalized care aimed at preventing or delaying cognitive decline and dementia.

For 20 years, Five Star Home Health Care has provided in-home nursing care, chronic disease management,  and restorative therapy to help older adults remain independent. Currently, it is enrolling patients in the Centers for Medicare & Medicaid Services (CMS) GUIDE program, which offers specialized dementia care services at no cost to traditional Medicare beneficiaries and their caregivers. These services include:

  • Personalized Dementia Care Plan and Family Caregiver Training
  • 24/7 Support Line
  • Care Coordination with a Dedicated Care Navigator
Through this collaboration, Sunday Health will provide cognitive evaluations for patients of Five Star Home Health Care. Patients diagnosed with MCI who do not qualify for the GUIDE program can receive ongoing care and support through Sunday Health's CognaMAP™ program, which is designed to prevent and delay cognitive decline. Patients diagnosed with dementia may enroll in the GUIDE program with Five Star Home Health Care and its partner, CareBrains. CareBrains develops and implements care plans and offers support groups and education programs for those in the GUIDE program through monthly family caregiver interactions.
